Below are links to declaration forms for Team managers to use at matches.
One set of blank copies will also be sent out with the pre-season mailing unless you have
indicated to the league secretary that you will use these copies.

They are in MS Word 97 format and are readable by Word 97 or later and Internet Explorer 5 or later.
They may be slow to load.

They can either be printed directly or saved to disk.
